Annual Block Program

One yearly fee. No surprise charges.

OHIP doesn't cover everything. Forms, notes, and prescription renewals requested outside a visit all fall outside provincial coverage. The Annual Block Program lets you pay one flat fee up front instead of a separate charge every time you need one.

What it is

Why some services cost money at a doctor's office

OHIP covers medically necessary visits, but it doesn't cover paperwork: forms, letters, and prescription renewals handled without an appointment. Under College of Physicians and Surgeons of Ontario (CPSO) rules, patients get a choice in how they pay for these uninsured services: one at a time as they come up, or bundled into an optional annual fee known as a block fee.

Enrolling is entirely your choice. If you'd rather not, you can keep paying for each uninsured service individually as it comes up, listed under "Pay as you go" below.

Why it helps

Built for patients who use more than one service a year

If you renew a prescription by fax, need a return-to-work note, or request a travel vaccination even a few times a year, those charges add up fast. The Annual Block Program covers a defined list of common requests for one flat rate, so you stop tallying up individual invoices.

It also waives one missed appointment fee and your first 20 pages of photocopying each year, two costs that pay-as-you-go patients don't get back.

Fees marked "+" vary with the complexity of the request. *Applies only to medications you've already been prescribed — new medications require an appointment.

Prescription Renewal Only

A lighter option if prescription renewals are the only uninsured service you tend to need. No doctor's appointment necessary if it's a refill of a medication you're already prescribed.

Prescription Renewal Plan$100.00

See how it adds up

Say over the year you need two return-to-work notes ($30 × 2 = $60) and two letters for private insurance ($30 × 2 = $60). That's already $120, the full cost of the Individual plan, and every other covered request that year, forms, certificates, notes, is included at no extra charge. Families juggling school forms, travel paperwork, and renewals for more than one person often cross that line even faster.

Not included, even with the block fee

Missed annual physical exam$150.00
Transfer of medical records$120.00+
Forms requested by employers or insurers$85.00+

These are billed separately whether or not you're enrolled in the program.
